Intensive Case Management (ICM)


About ICM
Our ICM program provides comprehensive, long-term support to youth (aged 16-29) and adults experiencing severe and persistent mental health challenges. Many of our clients experience substance-use health challenges, poverty, food insecurity, housing instability, and other health concerns in addition to their mental health diagnosis.
Our dedicated team of Community Support Workers collaborate with clients to develop goals and a plan for these individuals with the intent of eventually transitioning out of mental health services. Approximately 1,100 clients receive case management services through the Mental Health Support Services Collaborative (MHCSS).

Accessing ICM
To apply for Intensive Case Management services from Upstream Ottawa, or any other MHCSS partner agency in Ottawa, an application form must be submitted to MHCSS via the Canadian Mental Health Association - Ottawa Branch.
